Personalised care and support

Our dedicated and compassionate team are committed to providing person-centred care always focusing on tenants as an individual. We will work with tenants to identify their strengths and areas where we can help increase their independence, with an emphasis on helping you move towards independent accommodation.

We encourage tenants to develop their skills through a variety of activities, from work or volunteering, to attending college or other skills development courses. Working closely with other Multidisciplinary Teams we will support tenants to move on into their own home, accessing through Property Pool Plus. Your own self-contained flat which you can personalise to your own taste.

Support provided at Bluebell Lane can include assisting with shopping, attending appointments, emotional support, and support in maintaining a tenancy. Whether tenants need occasional assistance or more comprehensive support, we ensure that their care plans are adapted to their specific circumstances, allowing them to lead independent and fulfilling lives.

Tenancies are for up to two years and our support workers will work with tenants to work towards goal aspirations to move on before end the two years.

We will work closely with tenants to develop a comprehensive move on plan with a key emphasis of working on the areas our tenants would like to improve. The move on plan is reviewed regularly to achieve key milestones set along a tenant’s journey.
