Our Manchester floating support service provides person-centred, responsive and flexible care and support to people living with a mental health condition.
We work in partnership with you, your carers and families as well as other supporting agencies to provide you with the best possible care.
When we first meet you we will work together to complete a needs assessment form, this enables us to understand when and how much support you require. Once the assessment is completed we will draw up a support plan with you.
